DPC Membership Terms and Conditions

1. Overview

Upon enrollment in the DPC Membership, members receive direct access to Veronica Johnson, FNP-C, PMHNP-BC, for primary care and holistic health support. This membership provides unrushed, extended visits and a whole-person, root-cause approach to healing. Participants are responsible for scheduling their appointments using the designated scheduling app.

2. Payment Terms

The DPC Membership is a monthly subscription of $150 and can be canceled at any time before the next billing cycle. Payments are non-refundable once processed. White Oak Medicine reserves the right to adjust membership pricing with notice.

3. Inclusions

The DPC Membership includes:

  • Direct Primary Care (DPC) with a holistic, root-cause approach
  • Unrushed, extended 60 minute appointments 
  • Same-day/next-day availability
  • Direct access to your provider via call, text, or email—without intermediaries
  • Comprehensive physical and mental health care in one place
  • Medication deprescribing and safe tapering support when appropriate
  • All wellness services included at no extra charge, including:
    • The Morning Practice
    • Auricular Acupuncture
    • Community Process Groups
    • Breathwork Sessions
    • Deep Dives
    • Healing Library & Book Club
    • Tea Station & Herbal Apothecary access
    • Monthly Education Series & Friday Mocktails
  • Members-only pricing on herbs and supplements
  • Lab testing available as needed (prices vary and are billed separately)

4. Participation Requirements
Members are responsible for scheduling and attending their appointments. Missed appointments will not be refunded or credited. Please note that this membership is not intended for emergency or crisis care.

Scope of Care
White Oak Medicine provides holistic primary care with a focus on root-cause healing, prevention, lifestyle medicine, and chronic disease management. However, please be aware that we do not provide the following services: emergency or urgent care, gynecological exams, prostate exams, sutures, wound care or injury management, vaccinations, IV fluids, or injections. For urgent or emergency care, we encourage you to seek care at the nearest urgent care facility or hospital.

While we work to help you avoid the need for hospitalization whenever possible, we partner with an IV hydration business upstairs for hydration needs. Additionally, if you require sutures or other urgent medical care, there is an urgent care facility conveniently located right across the street.

5. Suitability
The DPC Membership is designed for individuals seeking holistic primary care, medication deprescribing support (where appropriate), and chronic condition management. It is not intended for emergency or crisis care and may not be suitable for individuals requiring urgent or intensive medical or psychiatric care.

6. Testimonials

7. Cancellations & Membership Changes

The DPC Membership is a month-to-month plan and can be canceled at any time before the next billing cycle. Refunds are not available for past payments. White Oak Medicine reserves the right to adjust program offerings, pricing, or schedules as needed.

8. Liability & Disclaimer

While White Oak Medicine provides comprehensive, holistic healthcare, participants acknowledge that healing outcomes vary and are not guaranteed. Members are responsible for their own health and well-being.

9. Confidentiality

All personal information disclosed during sessions remains strictly confidential.

10. Acceptance of Terms

Enrollment and payment in the DPC Membership signify full acceptance of these terms and conditions.
