The Hawthorn Session – Terms and Conditions

1. Overview
The Hawthorn Session is a one-time, standalone 2-hour comprehensive evaluation designed to provide clarity, insight, and direction. It may serve as a holistic second opinion, a starting point for care, or a clarifying consultation to determine appropriate next steps.

This session does not require enrollment in a membership or program. Some individuals may choose to proceed into ongoing care, structured programs, coaching, or medical services based on clinical recommendations and personal preference.

2. Payment Terms
The Hawthorn Session is billed at $750. Payment is required in full prior to scheduling. Payment plans may be available through third-party services (e.g., Afterpay), subject to their terms.

If you choose to enroll in an ongoing White Oak program or membership within a defined period after your Hawthorn Session, a portion of the Hawthorn Session fee may be applied toward your first month or program cost. The amount and eligibility are determined at White Oak’s discretion and may vary by pathway.

3. What the Hawthorn Session Includes
The Hawthorn Session includes:

  • A comprehensive review of your physical, mental, emotional, and functional health history

  • Discussion of current symptoms, stressors, patterns, and goals

  • Review of prior labs or records if submitted at least 48 business hours in advance

  • Clinical insight and recommendations regarding appropriate next steps, which may include lifestyle changes, coaching, functional testing, medical care, or structured programs

This session does not include:

  • Diagnostic testing or labs

  • A written treatment plan or roadmap

  • Ongoing messaging, follow-up care, or portal access beyond scheduling and administrative communication

4. Follow-Up and Access
Any additional support—such as lab review, decision-making, planning, or transition into care—requires a separately scheduled and billed follow-up visit or enrollment in a program or membership.

Between the Hawthorn Session and any follow-up appointment or enrollment, provider access is limited. Ongoing access is reserved for active patients enrolled in care.

5. Scheduling and Participation Requirements
Sessions are scheduled through White Oak’s secure scheduling platform.

All intake forms must be completed prior to the appointment. Relevant records or labs must be submitted at least 48 business hours before the session to allow for review.

By scheduling, you are reserving dedicated clinical time. Missed or late-cancelled appointments are nonrefundable.

6. Cancellations and Transfers
All Hawthorn Sessions are nonrefundable and nontransferable. White Oak Medicine reserves the right to modify scheduling or content as needed due to unforeseen circumstances.

7. Testimonials
Testimonials and shared experiences are provided for informational purposes only and do not guarantee specific outcomes. Individual results vary.

8. Liability and Scope
White Oak Medicine and Veronica Johnson, FNP-C, PMHNP-BC, provide clinical insight, education, and recommendations. No outcomes are guaranteed. Participation acknowledges that you remain responsible for your health decisions and outcomes.

9. Confidentiality
All personal and medical information shared through sessions and secure platforms is kept confidential in accordance with applicable privacy laws.

10. Acceptance of Terms
By scheduling and paying for the Hawthorn Session, you acknowledge that you have read, understood, and agreed to these Terms and Conditions.

This is not just an appointment.
It’s the beginning of a focused, clinical course of care.

The Hawthorn Path is White Oak’s functional medicine offering for people who need clarity, discernment, and a grounded next step. It is a time-limited, clinician-guided container, designed to quiet the noise and determine what truly needs attention.

This work begins with a comprehensive two-hour evaluation and unfolds over the course of 90 days.


What This Includes

Initial Evaluation (2 hours)
We will take the time to listen deeply and look at the full picture—physical symptoms, health history, emotional and nervous system patterns, rhythms, and lived context. This visit establishes the clinical foundation for the Hawthorn Path.

Ongoing Care (Every Other Week for 3 Months)
Following the initial evaluation, we meet every other week to interpret findings, sequence care thoughtfully, and make adjustments as your body responds.

When indicated, this may include targeted laboratory testing and specific dietary or supplement recommendations. Testing and supplements are recommended judiciously, discussed transparently, and billed separately.


What Happens After the Hawthorn Path

At the end of 90 days, we pause and reassess together.

From there, we decide collaboratively whether it makes sense to:

  • continue functional medicine care,

  • transition into Direct Primary Care,

  • engage in group-based rhythm work,

  • or complete care with a clear plan in hand.

There is no automatic renewal.

The Hawthorn Path is designed as a complete course of care, not a one-time consult. This container exists so we can move deliberately rather than reactively—and so decisions are guided by clarity, not urgency.
