Thyroid Health Assessment – Terms and Conditions

1. Overview

This is a one-time, 1-hour individual consultation with Veronica Johnson, FNP, as part of White Oak Medicine’s Direct Primary Care & Functional Wellness services. This session is designed to explore your thyroid-related symptoms, health history, and root-cause factors contributing to your current concerns.

2. Payment Terms

The total cost for this 1-hour assessment is $97. Payment is due at the time of registration. Your session will not be confirmed until payment is received.

3. What’s Included

This assessment includes:

• A 1:1 session with Veronica Johnson (up to 60 minutes)

• Review of your symptoms, current medications/supplements, and any recent labs

• A holistic interpretation of your thyroid function and contributing factors

• Personalized next steps for your care (which may include labs, lifestyle shifts, or further evaluation)

4. Participation Requirements

Once you schedule your appointment, that time is reserved just for you. If you are unable to attend, please provide at least 24 hours' notice to avoid cancellation. Missed appointments or late cancellations (less than 24 hours' notice) are non-refundable and not eligible for rescheduling.

6. Cancellations and Rescheduling

You may reschedule your appointment up to 24 hours in advance using the scheduling link provided. If you cancel or do not attend your session without giving 24 hours' notice, the session will be forfeited and no refunds or credits will be issued. Veronica reserves the right to modify availability or reschedule in the rare case of an emergency.

7. Liability and Disclaimer

This session is not a substitute for emergency care or ongoing primary care. Veronica Johnson is a licensed Family Nurse Practitioner providing holistic and functional wellness services. By participating, you agree to take full responsibility for your health decisions and outcomes.

8. Confidentiality

All personal health information shared during this session will be kept private and confidential in accordance with HIPAA guidelines.
