Direct Primary Care & Functional Wellness Services - Individual Session Terms and Conditions

1. Overview

An initial 1.5 hour evaluation is required for all individual sessions with Veronica Johnson. This is the first session to begin services and will lay the foundation for your personalized care plan.

2. Payment Terms

The total cost for this initial 1.5 hour assessment and is $297. Payment is required before scheduling your session.

3. Session Inclusions

The initial session includes:

• A 1.5 hour assessment to explore your full health history, current symptoms, and mind-body connection.

4. Participation Requirements

Participants are expected to attend sessions that they schedule with Veronica using the provided scheduling platform. Full participation is expected, but to the extent that feels appropriate for you. A refund or credit will not be issued if you miss your scheduled session. By scheduling and paying upfront, you are committing to the time and date you select.

5. Testimonials

Testimonials provided by previous participants are for informational purposes only and do not guarantee similar results for all participants. Outcomes vary depending on individual circumstances.

6. Cancellations and Transfers

Veronica Johnson reserves the right to modify content or session schedules as needed. In the event of a cancellation by the participant, refunds or credits will be determined based on the timing of the cancellation notice. No credits will be made for appointments cancelled less than 48 hours before the scheduled session.

7. Liability and Disclaimer

While every effort is made to provide valuable and accurate guidance, Veronica Johnson is not liable for any personal outcomes or lack thereof. Participants are responsible for their own well-being and must acknowledge this before beginning services.

8. Confidentiality

All personal information shared within individual sessions will be kept confidential. Participants are expected to maintain confidentiality regarding other participants' experiences in any group programs or services offered.
