White Oak Medicine Membership Terms and Conditions
1. Overview
The White Oak Care Pathways (Redbud, Mugwort, Yarrow, and Motherwort) provide tiered levels of holistic direct primary care and functional wellness, tailored to your individual support needs. Each membership includes a combination of personalized medical visits, nervous system regulation support, and access to White Oak Medicine community offerings. Memberships are designed to provide steady rhythms of care, not one-time or short-term services.
2. Payment Terms
Startup Payment: Your first payment includes the Initial Comprehensive Evaluation (required for all new members) and your first month of membership.
- Redbud: $520 total to get started, then $147/mo
- Mugwort: $545 total to get started, then $297/mo
- Yarrow: $622 to get started, then $497/mo
- Motherwort: $797/mo (no additional startup fee; evaluation included)
After this first payment, your card will be automatically charged the monthly membership fee on the same calendar day each month.
Payment must be received before scheduling your initial session.
All membership payments are non-refundable and non-transferable.
3. What’s Included (General)
- 1:1 visits with your provider (frequency varies by membership tier).
- Nervous system regulation support woven into every visit.
- A custom Plan of Care rooted in whole-person medicine.
- Direct access to your provider via secure messaging.
- Unlimited auricular acupuncture during 1:1 sessions.
- Free attendance at White Oak Health Education Series classes.
- Invitations to White Oak Socials (“Healthy Happy Hours”).
- Optional lab ordering, including functional medicine labs, supplement recommendations, and medication management as clinically indicated, and for an additional cost (insurance may cover some labs if applicable).
Specific frequency of visits is outlined in each membership tier description (e.g., Redbud = 4–6/year; Mugwort = monthly; Yarrow = twice monthly; Motherwort = weekly).
4. Visit Scheduling & Frequency
- Visits are intended to be used on a regular, ongoing rhythm.
- Visits may not be “banked” or used all at once.
- This plan is designed to support steady care, not short-term intensives.
- If your needs shift and you require more frequent care, you may upgrade to a higher tier for additional support.
5. Minimum Commitment & Tier Changes
- All memberships require a minimum commitment of 3 months.
- After the initial 3-month period, you may cancel at any time before your next billing cycle.
- If you find that you need additional support, you may upgrade to a higher tier at any time, including during your initial 3-month commitment.
- To downgrade to a lower tier, we ask that you first honor your 3-month minimum commitment at your current tier before making the transition.
6. Participation Requirements
- Participants are expected to schedule and attend sessions using the White Oak scheduling platform.
- Payment secures your time; refunds or credits will not be issued for missed appointments.
- By scheduling and paying upfront, you are committing to the time and date you select.
7. Testimonials
Testimonials provided by previous participants are for informational purposes only and do not guarantee similar results. Outcomes vary depending on individual circumstances.
8. Cancellations and Transfers
White Oak Medicine reserves the right to modify session schedules or content as needed. All memberships are non-refundable and non-transferable. In the event of a cancellation by the participant, refunds or credits will not be issued.
Membership cancellations must be completed via your Kajabi account prior to the next billing cycle. If you run into any issues with this step, please reach out for support. Once a payment has been processed, no refunds or credits will be provided.
9. Liability and Disclaimer
While every effort is made to provide valuable and accurate guidance, Veronica Johnson and White Oak Medicine are not liable for any specific personal outcomes. Participants are responsible for their own well-being, and payment reflects this acknowledgement before beginning services.
10. Confidentiality
All personal information shared within sessions will be kept confidential. Participants are expected to maintain confidentiality regarding other participants’ experiences in any group programs or services offered.
