This track begins with a two-hour intake where we lay the groundwork for ongoing holistic primary care. You’ll leave with clear next steps and a “mini roadmap” tailored to your immediate needs.

Ongoing care continues with:

Seasonal or quarterly visits

Nervous system support woven into care

A safe medical home without unnecessary testing or fear-driven protocols

This path is ideal if you:

Want ongoing holistic primary care

Need a provider who truly listens and understands

Prefer seasonal check-ins and gentle support

Are working with a therapist or taper coach and need a respectful prescriber

2) The Functional Medicine Path 

Root-cause healing, guided by plant allies

This track offers three membership levels, tailored to the depth of care and support you need in this season of your life.

Memberships include:

Direct access to your provider

Same-day or next-day appointments

Whole-person care (medical, psychiatric, therapy, and functional labs at additional cost)

Discounts on supplements through Fullscript

Reduced rates on labs and White Oak wellness offerings

This path is ideal if you:

Want to uncover the root causes of chronic symptoms

Need more frequent connection and accountability

Are tapering medications or moving through a major transition

3) The One Time Consult Path (The Hawthorn Sessions)

A one-time consult (or holistic second opinion) for clarity and direction

The Hawthorn sessions are available as a stand-alone offering for those who want clarity without ongoing membership.

They include:

A two-hour comprehensive evaluation

A personalized written care plan

A one-hour follow-up session to review and integrate your plan

This is ideal if you:

Are seeking a holistic second opinion

Want clarity on a diagnosis, treatment plan, or next steps

Prefer to take the plan and implement it on your own

4) Safe Tapering and Deprescribing (The Dandelion Path)

Gentle oversight for those already tapering

This track is designed for people who are already working with a therapist or taper coach and simply need prescription oversight. It includes:

One visit every three months

Coordination with your taper coach

A referral-based, low-frequency rhythm of care

This path is ideal if you:

Are actively tapering psychiatric medications

Already have a taper coach but need a prescribing provider

Want safe, respectful oversight without unnecessary visits
